For Students
A student application includes all of the following:

  • Completed online application form
    (click for sample)
  • Personal statement of career goals
    (500 words)
  • Letter of good standing from current medical school
  • Letter of recommendation
For Mentors
A mentor application includes all of the following:

  • Mentor form
    (click for sample)
  • NIMH formatted biosketch
  • List of all past and current trainees
